Patient journey

20M, I headed to the ER Sunday morning with brutal pain through my lower back and abdomen. They came back with a 3mm kidney stone, and about four hours into my hospital stay the pain just cut off and i’ve felt fine ever since, apart from a little tightness in that spot. I’m trying to figure out whether going back to work is a smart move? I’m also worried about what I should do when things inevitably turn bad again. Is peeing blood by itself reason enough to head back to the hospital or urgent care? I’ve heard that’s pretty common with kidney stones but I don’t want to gamble on an infection. Also, what does it mean when the pain sticks around in my lower back? My assumption is that as long as I can urinate then I should just keep dealing with it. I guess what I’m really asking is what level of pain and symptoms counts as normal enough to just tough out on the pain meds. Please share whatever advice you’ve got.

edit: also, i figure the answer is yes, but will the pain hit as hard as it did the first time?

Get 3 liters of water down through the day, flomax would help, your primary health care provider can write you that, it gets things moving. I would go back to work without question. Blood is normal woth stones that are moving along. Basic painkillers traded off with ibuprofen, assuming you don’t have contraindications, is a solid plan. Percentage wise the odds of passing a stone that size sit in the mid 90s.

Yikes, 3mm stones could go either direction -- a few clear out by themselves like yours may be doing, while others lodge in place and hurt like hell down the line. Blood in the urine is typical with stones, though if you're pushing out clots, running a fever, or the pain comes roaring back, that lands in urgent care territory. Going to work is likely fine given you feel okay at the moment, just keep the fluids up and don't brush off your body if anything shifts.

My first one was 3mm and it dragged on three months. I kept working through all of it, though that one was mostly just uncomfortable. The most recent was 5mm and it came out in 3 days, but the pain left me completely useless. No chance I could have worked. They're so unpredictable. I've had tiny stones that brought on bloody urine with no infection or any pain. That scared the hell out of me. Hoping you’re already past the worst of it and that sucker moves fast.
